The information we are collecting includes basic contact information and characteristics about your congregation, plus information about the types of environmental education, restoration, or advocacy efforts in which your congregation has engaged.

You will have the opportunity to share your name and contact information to help us better connect with you. Responses on the Intake Form will only be used for internal purposes.

It may be helpful to download the list of questions so that you can collect the information from your congregation as needed.
